There is no official answer for this. You will find that one hosts idea of managed is different to another hosts idea of managed. It's best to just read up what exactly is included when you buy a server and ignore the word fully and partially and just read what they actually do offer.

That's the best advice. Also, read carefully the Terms of Service of each provider and you'll have a closer idea of what you might expect from them. Regards, Jaime
