A CDN is always helpful, irrespective of whether you're on a shared or dedicated server. It not only reduces the load on server, it helps cache files closer to user, resulting faster load times.

1. Pretty much the same. 2. Majority of the large websites use some sort of CDN. With CDN providers like Akamai etc. you wouldn't even know most of the time, that the website was using a CDN.

1. Exactly the same, CloudFlare is per domain not per server. 2. CloudFlare is not the definitive answer to a fast website. The server still needs to be properly configured. optimised, server side caching, etc etc.. lots of variables.

Cloudflare setup should be the same, it does not matter where you host your website, on a shared hosting or on a dedicated server. Indeed, a lot of big websites are using their services, they might host some static subdomains to serve their JS, CSS, it's pretty common. Cloudflare helps a lot to increase the website performance. And they provide free plan.

Yes, unless you have deep pockets to afford top end CDNs like Akamai.

Yeah if you have that popular of a site then you should be using a CDN to offer better website performance to our end users and reduce the load on your server. If you are going from shared hosting to a dedicated server though make sure you understand how to setup the dedicated server or ask your hosting provider for help if they are managing the server for you as a CDN is not the only thing you should be utilizing. You need a well configured server as to perform well, so get a system admin if you're not sure.
